Left leg swelling for 2 weeks just after hysterectomy

POCUS in I – AIM approach

Device: Vscan with dual probes

Indication: suspect DVT

Acquisition:
1. 3 point scan at inguinal area, mid-thigh and poplital fossa
2. Start from curve probe for larger screening, then change to linear probe
3. Compression is very important. Completely compressible vein indicates patency

Interpretation:
Left inguinal scan with Vscan (linear part)

  1. Echogenic thrombus within left common femoral vein
  2. Non-compressible

Making decision:

  1. Comfirm DVT (Diagnosis)
  2. Start anticoagulants (Treatment)
  3. Admit for continuing treatment and comprehensive vascular sonography (Disposition)
